Good Faith Estimate Notice
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.

WHAT ARE YOUR FEES?
Abundant Life Counseling offers a range of options for session rates that vary based on the service you are seeking. Our rates are comparable with Masters level specialist therapists in the tri-county area. These rates apply to both virtual and in-person place of service locations.
Diagnostic Assessment (up to 1 hr) - $185
Psychotherapy for Adult, Teen or Child (53 minutes) - $150
Couples & Family Counseling (26-50 minutes) - $150
EMDR Intensive (3 hours) - $450
Couples Intensive (6 hours) - $935
Mindset Coaching, Parent Coaching and Relationship Coaching (50 minutes) - $85

DO YOU TAKE MY INSURANCE?
We are in network with Anthem, Blue Cross/Blue Shield, Medical Mutual of Ohio, Aetna, Cigna and Optum (UMR, UBH and UHC) networks. ***We do NOT have network contracts with EAP, Medicaid or Medicare.***
If you have medical insurance, we would like to help you receive your maximum allowable benefits; therefore we ask that you provide your insurance information for your first appointment and when there are any changes in your insurance coverage. Additionally, we require that each patient pay their deductible, copay, and/or coinsurance payment at the time service is provided.
We are an out-of-network provider with most other PPO Plans. Payment is made by the client at the time of the session, and if you choose, insurance will reimburse you a percentage of the session cost according to your plan’s OON benefits. Insurance providers typically reimburse 40-80% of session costs. Insurance plan coverage varies widely, so please check with your provider via phone or online for specific coverage and rates of reimbursement.
Helpful questions to ask your insurance company:
Do I have mental health coverage?
What percentage of my bill will be covered for services?
How many sessions will be covered per calendar year?
What is my deductible and has it been met?
If you choose to seek reimbursement, you can do this with a "superbill" that lists your diagnosis and other necessary information. We can provide you with a superbill weekly or monthly for you to submit to your insurance for reimbursement.
Our clients prefer a personalized higher level of service that is not dictated by the tight restraints of in-network managed care. With us, you control your confidentiality and whether your information is released to your insurance. Insurance providers require a diagnosis for payment and keep records of treatment, which may not be beneficial to some of our clients based on their profession or other factors.
DO YOU OFFER SLIDING SCALE?
We do not offer a sliding scale due to the high demand for our therapists and limited hours available. However, we offer opportunities to learn and engage via community events and speaking engagements free of charge.
WHAT FORMS OF PAYMENT DO YOU ACCEPT?
We accept all major credit cards, checks, cash, as well as HSA, FHA and FSA cards. We offer easy online payment options as well as automatic withdraw options.
